A NYIA assessment is an evaluation conducted by the New York Independent Assessor. NYIA is a state-contracted entity responsible for completing independent assessments for individuals seeking Medicaid-funded long-term Home Care services in New York.
If someone is applying for PCA or HHA Home Care through Medicaid, especially under an MLTC or MAP plan, a NYIA assessment is usually required before services can be approved. The purpose of the assessment is to determine whether the individual meets medical necessity requirements and to evaluate the level of assistance they need with daily activities.
The NYIA assessment is not performed by the Home Care agency. It is conducted by an independent nurse assessor, either in person or via telehealth when allowed. This separation is intentional. The state uses an independent evaluator to ensure that eligibility decisions are objective and based on documented functional limitations rather than preference.
During the assessment, the nurse evaluates the patient’s ability to perform activities of daily living. This includes bathing, dressing, toileting, transferring, walking, eating, and overall safety awareness. The assessor will ask detailed questions about daily routines, mobility, cognitive status, fall risk, and medical history. The nurse may also review medical records and speak with caregivers present in the home.
It is important to understand that a diagnosis alone does not guarantee approval. The NYIA assessment focuses heavily on functional impact. The key question is whether the patient requires ongoing hands-on assistance or supervision to remain safely at home. If functional limitations are not clearly demonstrated, Home Care may not be authorized even if medical conditions exist.
The results of the NYIA assessment are then used by the Medicaid managed care plan, such as an MLTC or MAP plan, to determine the number of authorized hours and the appropriate level of care. In many cases, without a completed NYIA assessment, services cannot begin.
