Once the eligibility form is submitted, our team reviews the information and assigns an intake specialist to the case. The specialist contacts the family to confirm details, explain the following steps, and provide support throughout the process.
The patient will complete two required evaluations:
• a social assessment
• a nursing assessment
These evaluations help determine daily living needs, medical requirements, and the level of home care support the patient may qualify for.
After the assessments are completed, the Medicaid Long Term Care plan reviews the results and determines the amount, frequency, and duration of services. These recommendations are based on the patient’s health status and functional needs.
