
Managed Long Term Care Plan (MLTC) provides quality health care services to Medicaid-eligible adults in need of Community-Based Long Term Services and Support. MLTC offers access to benefits and services that help members live healthier, more productive lives. A team of healthcare professionals works collaboratively with members, their families/representatives, and their providers to create a plan of Care tailored to the specific needs of each member. Every member is assigned a care management team whose responsibilities include coordinating healthcare services, conducting home visits to monitor health status, and assisting with social and environmental services.
WHO IS ELIGIBLE FOR THE MANAGED LONG-TERM CARE PLAN (MLTCP)?
Eligibility depends on several criteria, which include but are not limited to the following:
- Must be at least 21 years old.
- Must live in New York State
- Must need more than 120 days of Community-Based Long-term Care Services.
- Must be Medicaid eligible
