If you meet the following criteria, we can assist you with enrolling in Medicaid:
- 18 Years or older.
- Need help with day-to-day activities because of a disability or chronic health issue.
- Capable of managing caregivers, or having a representative who can do so.
- Reside in our New York service area, and have or wish to apply for Medicaid.
