After you submit the form and provide us with the necessary information, we will review the patient’s eligibility and assign one of our intake specialists to the case. They will contact you to discuss the next steps and answer any questions you may have.

The assigned coordinator will help the patient who requires care complete a social and nursing assessment. This assessment will evaluate the patient’s health status and determine their eligibility for home care services.

Based on these assessments, the Medicaid Long Term Care plan will recommend the appropriate amount, frequency, and duration of services for the Medicaid recipient.

Family Caregiver Requirements:

PCA Family caregivers must be at least 18 years old and authorized to work in the United States to participate in the program. Additionally, they must acquire a Personal Care Aide (PCA) license and consent to an initial mandatory health assessment, per the New York Department of Health.
